Check You Live Within The Boundary Area

Please note: we will only be able to register patients who fall into our catchment area.

This is generally speaking the area to the west of the following roads: Lower Bognor Road, Aldwick Road, and Aldwick Street

 

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill.

If you are already a patient and are about to move to a new address please let the reception team know by completing our change of details form. They can decide whether or not you are still within our catchment area. If you are not, and you would like to stay with the practice, please make an Out Of Area Request. All cases will be dealt with on an individual basis, by the practice manager and your named doctor.

GP2GP – The Electronic Transfer of Patient Records

We are able to receive most computer held patient records electronically from your previous surgery so they arrive much more quickly than paper notes. This is called the GP2GP electronic transfer of patient records. The paper notes will continue to be sent via an NHS delivery service.

Book a New Patient Health Check

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.

Temporary Services

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.

Your Named GP

As part of the NHS commitment to providing more personalised care, all practices are required to provide all their Patients with a named GP who will have overall responsibility for the care and support that our surgery provides.

Please note that:

  • This will not impact your experience at the practice, the provision of appointments, your treatment, or which GP you can see.
  • You may wonder why your allocated GP is not necessarily the one you see most regularly.  Please be assured that you all other clinicians can provide the care you need and have access to the same medical record your named GP has.
  • Having a named GP does not guarantee you will always be seen by that GP.
  • If you wish to be registered with a different GP we can alter this for you. However, this does not change the availability of appointments for that GP.
  • Please note that the GP responsible for your care may be subject to change and reallocation in the future
  • You do not need to take any further action, but if you have any questions or wish to know your named GP, please speak to our Reception team.

What does 'accountable' mean?

This is largely a role of oversight; it is to reassure patients that they have one GP within the practice who is responsible for ensuring that this work is carried out on their behalf.  

What are the named GP's responsibilities to 75s and over?

For patients aged 75 and over the named accountable GP is responsible for:

  • Working with relevant associated health and social care professionals to deliver a multi-disciplinary care package that meets the needs of the patient
  • Ensuring that these patients have access to a health check as set out in section 7.9 of the GMS Contract Regulations.

Does this mean 24-hour responsibility for patients?

No. The named GP will not:

  • Take on vicarious responsibility for the work of other doctors or health professionals
  • Take on 24-hour responsibility for the patient, or have to change their working hours. The requirement does not imply personal availability for GPs throughout the working week
  • Be the only GP or clinician who will provide care to that patient

Can patients choose their own named GP?

In the first instance, patients should simply be allocated a named GP. However, if a patient requests a particular GP, reasonable efforts should be made to accommodate their preference, recognising that there are occasions when the practice may not feel the patient's preference is suitable.

Do patients have to see the named GP when they book an appointment with the practice?

No. Patients can and should feel free to choose to see any GP or nurse in the practice in line with current arrangements. However, some practices may see this as a way to encourage and promote a greater degree of continuity of care for patients.
